Step by step process for NEW patients to engage with myMDadvocate
Return patients please log in via my SimplePractice portal or see my Return Patients page for further instructions.
- Contact myMDadvocate via email or phone with first & last name, phone number, referral source (ex. found website, friend mentioned, physician referred, etc.) and best times to receive a phone call. Be sure to check your email Junk folder for email replies! Note: only WA state residents who will physically be in WA state at the time of their video visit can use this service (per federal and state Telehealth regulations).
- Dr. Baumgaertel will set up a 10-15 minute free phone session to discuss mutual goals of working together.
- Your myMDadvocate account will then be set up in SimplePractice, the secure online business management platform for all visits, communication and transactions.
- You will receive an email link to sign into myMDadvocate’s SimplePractice Client Portal to review business policies, complete intake forms, and enter basic information:
- The Release of Information Consent allows Dr. Baumgaertel to communicate with your physician(s), if desired, and to access your Epic Electronic Health Record (EHR) data if needed. This form is renewed annually.
- The Demographics Information and Insurance Opt Out/Do Not Bill forms need to be completed even though myMDadvocate does not bill your insurance. This is in the event that a “Superbill” is generated (based on individual request) and also to conform with Medicare/CMS rules regarding self-pay.
- The following Consent Documents are also required by law in WA state: Notice of Privacy Practices, Practice Policies, Consent for Telehealth Consultation, Credit Card Authorization.
- Credit card information is required to enable online payments via Stripe, and autopay is selected for efficient payments.
- Intake Questionnaire completion is also required before your first video visit.
- After the above steps are completed you will be able to request your own appointments directly on myMDadvocate’s SimplePractice calendar. If you need an appointment before 48 hours, you’ll need to contact Dr. Baumgaertel via Secure Messaging which is accessed via the Client Portal under the Communication tab. You will receive a courtesy reminder email 48 hours prior to your appointment.
